All services provided by Wellness Reimagined LLC and by Roxane Sazegar, FNTP, RHP (“Practitioner”) are for educational and informational purposes only. Nothing provided—whether written, verbal, or delivered in consultation or through lab interpretation—constitutes medical advice, medical diagnosis, medical treatment, or the practice of medicine.

The Practitioner is not a medical doctor, nurse, physician assistant, registered dietitian, or licensed healthcare provider. No recommendations offered should be interpreted as medical advice or as a substitute for consultation with a licensed physician or other qualified medical professional.

You agree to consult your physician before making any changes to diet, supplements, medications, or lifestyle practices. You also agree to seek immediate medical attention if you experience concerning or worsening symptoms.

1. Nature of Services

Services offered by Wellness Reimagined LLC may include:

  • Nutritional therapy

  • Functional nutrition coaching

  • Lab test facilitation (including MRT, GI-MAP, blood chemistry panels, and other functional labs)

  • Functional interpretation of lab results

  • Personalized nutrition, lifestyle, and supplement recommendations

  • One-on-one coaching or consulting sessions

  • Educational materials

  • Life coaching

These services are for educational purposes only. The Practitioner does not diagnose, treat, cure, or prevent disease and does not replace the care of a licensed medical provider.

2. Not Medical Advice / Scope of Practice

By purchasing any service, you acknowledge that:

  • The Practitioner does not provide medical evaluation, diagnosis, or treatment.

  • All recommendations are non-medical and educational in nature.

  • You must consult a licensed healthcare provider before implementing any recommendations.

  • You are solely responsible for your own health decisions, actions, and outcomes.

  • Any changes to medications, supplements, or treatment plans must be supervised by your medical provider.

If you experience unexpected or concerning symptoms, you agree to contact your healthcare provider immediately.

3. Lab Testing, Fees, and Limitations

For any lab-related purchase:

  • Fees cover only the items explicitly listed in the product description at the time of purchase.

  • Blood draw fees charged by laboratories or phlebotomy sites are not included unless specifically stated.

  • Lab turnaround times and operational issues are outside the Practitioner’s control.

  • Lab results are interpreted from a functional nutrition perspective only and are not intended to diagnose or rule out any medical condition.

If additional analysis, follow-up sessions, or extended support are requested beyond what is included in the purchase, additional fees will apply.

4. Payments, Refunds, and Service Expiration

  • All payments made through Stripe are final and non-refundable, except where required by law.

  • You are responsible for the full purchase amount, whether or not you complete the sessions or use the materials and services.

  • Services must be used within the time frame stated in the product description; unused services expire and cannot be refunded, extended, or transferred unless expressly stated.

  • Certain programs may offer extensions for an additional fee.

  • If a payment plan is offered, you agree to complete all payments; failure to do so may result in discontinued services.

  • If you authorize Stripe to store your payment method, you agree that Wellness Reimagined LLC may charge the stored payment method for future authorized transactions related to your services, including payment plans, extensions, or invoiced services.

5. Scheduling, Cancellations, and No-Shows

  • Sessions must be scheduled based on Practitioner availability.

  • A minimum of 24 hours’ notice is required to reschedule.

  • Late cancellations or missed appointments may result in a $50 fee or forfeiture of the session, depending on the service purchased.

6. Client Responsibilities

By engaging in services, you agree to:

  • Provide accurate and complete health information

  • Communicate openly about symptoms, concerns, and changes in health status

  • Follow functional recommendations to the best of your ability

  • Seek medical care when necessary

  • Inform both your physician and the Practitioner of adverse reactions or significant health changes

You understand that your results depend on your effort, consistency, and individual bio-individuality.

7. Services for Minors

Services provided to individuals under the age of 18 are offered only with the knowledge and consent of a parent or legal guardian.

A parent or legal guardian must complete all purchases, agreements, and consent on behalf of any minor and is solely responsible for the minor’s participation and implementation of recommendations.

8. Confidentiality

The Practitioner will maintain the confidentiality of your personal and health information, except:

  • When disclosure is required by law

  • When you grant written permission

  • When sharing anonymized (de-identified) information for educational or professional use

9. Limitation of Liability

To the fullest extent permitted by law:

  • The Practitioner is not liable for any adverse effects or outcomes resulting from your decisions or actions.

  • You assume full responsibility for your participation and health choices.

  • The Practitioner’s total liability for any claim shall not exceed the amount you paid for the specific service in question.

You agree to indemnify and hold harmless the Practitioner and Wellness Reimagined LLC from any claims arising from your participation.

10. No Guarantee of Results

The Practitioner does not guarantee:

  • Symptom improvement

  • Diagnosis or resolution

  • Specific outcomes

  • Medical changes

Results vary widely based on individual factors beyond the Practitioner’s control, including the Client’s level of effort towards implementing change.

11. Governing Law and Dispute Resolution

These Terms are governed by the laws of the State of Texas. Any dispute shall be resolved by binding arbitration in Harris County, Texas, under the Commercial Arbitration Rules of the American Arbitration Association (AAA). The arbitrator’s decision is final and enforceable.

12. Updates to Terms

Wellness Reimagined LLC may update these Terms at any time without prior notice. The version posted at the time of your purchase applies to your transaction.

13. Acceptance of Terms

By completing a purchase through Stripe, you confirm that:

  • You have read and understand these Terms of Service

  • You agree to be bound by them

  • Your purchase acts as acceptance in lieu of a signature

If you do not agree, do not complete your purchase.
